Cervical screening (HPV test)

Almost every country in the region has switched from looking for abnormal cells to looking for the virus that causes them. The test is better, so the gap between tests is longer — and in several countries you can now take the sample yourself.

The switch from cells to virus

Cervical screening used to mean cytology: a sample of cells scraped from the cervix, spread on a slide and examined for changes. It worked, and it drove cervical cancer down dramatically over decades, but it looks for damage that has already happened and a single sample misses a fair amount of it. Primary HPV testing looks instead for the high-risk human papillomavirus types that cause virtually all cervical cancer. The large European randomised trials, pooled by Ronco and colleagues, found HPV-based screening gave substantially better protection against invasive cervical cancer than cytology. Estonia, Norway, Sweden, Latvia and Denmark have all moved to it, and Finland uses it widely.

Why a longer interval is not less care

A negative HPV test is a much stronger reassurance than a negative smear, because it says the cause is absent rather than that the damage is not yet visible. That is why intervals lengthened when the test changed — five years is standard from 30, and Sweden lengthens it further in the older bands. People often read a longer gap as a cut. It is the opposite: the test is good enough that testing more often would mostly generate false alarms, because an HPV infection picked up at 28 will usually have cleared by itself by 30.

Taking the sample yourself

Self-sampling is the biggest practical change in this programme in a generation. A kit posted to your home, a swab you use yourself, a prepaid envelope back to the laboratory. Estonia offers it, and it exists in several forms elsewhere in the region. It exists because the women least likely to attend a speculum appointment are the women at highest risk, and a self-taken HPV sample performs well enough that reaching them matters more than the small difference in sample quality. A positive self-test is followed by a clinic examination; a negative one ends the round.

What a positive HPV result means

Mostly, it means you have a common infection. HPV is very common and the large majority of infections clear without doing anything at all. A positive result is triaged — usually by examining the same sample for cell changes — and depending on what that shows you are either re-tested in a year or referred for colposcopy, where the cervix is examined under magnification. Colposcopy is an outpatient appointment, not surgery. Cell changes that need treating are removed in a short procedure, and treating them is what prevents the cancer.

Vaccination does not replace it

HPV vaccination protects against the types responsible for most cervical cancer, and the vaccinated cohorts now reaching screening age have far less disease. It does not cover every high-risk type, and screening programmes across the region continue to invite vaccinated women. The likely future is fewer tests over a lifetime for vaccinated women rather than none, and the countries in this region will change their programmes at different speeds, as they did with HPV testing itself.

Dažnai užduodami klausimai

Why is cervical screening now every five years instead of three?

Because the test changed. A negative HPV test rules out risk for much longer than a negative cell sample, so the interval was lengthened to match the strength of the result. Testing more often would mostly detect infections that would have cleared on their own.

Can I take the cervical sample myself?

In several countries in the region, yes. A self-taken HPV kit can be posted to you, used at home and returned to the laboratory. A positive result is followed up with a clinic examination; a negative result completes the round.

I tested positive for HPV — do I have cancer?

Almost certainly not. HPV is a common infection and most infections clear by themselves. A positive test triggers triage — usually a look at the cells in the same sample — and either a repeat test in a year or a colposcopy appointment, which is an examination, not an operation.

Do I still need screening if I have had the HPV vaccine?

Yes. The vaccine covers the types that cause most cervical cancer but not all high-risk types, and every programme in the region continues to invite vaccinated women. Intervals may lengthen for vaccinated cohorts in future, at different speeds in different countries.

Breast cancer screening (mammography)

An X-ray of each breast, read by two radiologists, repeated every two years across a band of ages that starts at 40 in Sweden and 50 almost everywhere else. What it finds, what it misses and what a call-back means.

Bowel cancer screening (stool test)

A sample from one bowel movement, taken at home and posted. It is the only screening programme in the region that routinely prevents cancers rather than only finding them early — because the follow-up removes the polyp.

Prostate screening (PSA)

Latvia and Lithuania fund PSA testing for men in their fifties and sixties. Sweden assessed the same evidence and recommends against an organised programme. Here is what the trials actually found.

Lung cancer screening (low-dose CT)

The trials are positive and the European recommendation asks countries to build programmes stepwise. In this region it is still pilots rather than invitations — but the eligibility rule is worth knowing.

Abdominal aortic aneurysm scan

The only non-cancer screening programme in the region. Sweden offers men a single abdominal ultrasound in the year they turn 65, looking for a silent widening of the main artery.
