Bowel cancer screening (stool test)

A sample from one bowel movement, taken at home and posted. It is the only screening programme in the region that routinely prevents cancers rather than only finding them early — because the follow-up removes the polyp.

The test is easier than its reputation

The modern faecal immunochemical test needs a scraping from a single bowel movement, collected on a small stick that goes into a tube of preservative and then into a prepaid envelope. There is no diet to follow beforehand — that was the old guaiac test, which reacted to blood from steak as readily as blood from a polyp. FIT uses an antibody specific to human haemoglobin, so what it detects came from your own gut. The commonest reasons a kit is never returned are embarrassment and forgetting, not difficulty.

What it is really looking for

Not cancer, and not exactly blood you can see. It measures very small amounts of haemoglobin in stool, because bowel tumours and larger polyps bleed intermittently long before anything is visible. Intermittently is the operative word: a polyp that did not bleed on the day you sampled produces a negative test. That is the main limitation of the whole approach, and it is why the programmes repeat it every one or two years rather than once.

Why this programme prevents cancer

Every other screening programme in this list finds disease earlier. Bowel screening can stop it happening. A positive test leads to a colonoscopy, and at colonoscopy an adenoma — a benign polyp that might have become a cancer over the following decade — is removed there and then. That is prevention in the literal sense. The Cochrane review of the faecal occult blood trials found around a 16 per cent reduction in bowel cancer deaths among everyone invited, and a larger reduction among those who actually took part.

A positive result, and what happens then

Most positive tests are not cancer. Haemorrhoids, a fissure, inflammation, a nosebleed swallowed the night before, or a polyp that will turn out to be harmless all produce blood. A positive result is an appointment for colonoscopy: a day with bowel preparation that most people find worse than the procedure itself, then a camera examination usually done under sedation. If a polyp is found it is removed and sent to the laboratory, and you are told when your next colonoscopy or screening round should be. Declining the colonoscopy after a positive test wastes the entire benefit of having done the test.

The bit the programme does not cover

Screening is for people with no symptoms. Blood you can see, a persistent change in bowel habit lasting weeks, unexplained weight loss, or iron-deficiency anaemia are not screening questions and should not wait for the next round — they are reasons to see a doctor now, regardless of when your last negative stool test was. A negative FIT does not overrule a symptom.

Dažnai užduodami klausimai

Do I need to follow a diet before the bowel screening test?

No. The modern immunochemical test detects human haemoglobin specifically, so meat, vitamin C and horseradish no longer interfere. Dietary restriction belonged to the older guaiac test.

My stool test was positive — is it cancer?

Most positives are not cancer. Haemorrhoids, a fissure, inflammation or a benign polyp all bleed. A positive test earns you a colonoscopy, and going to it is the whole point of having done the test.

How often should the bowel test be repeated?

Every one or two years depending on the country: Latvia offers it yearly, most of the region every two years. Repetition matters because a polyp bleeds intermittently and may simply not have bled on the day you took the sample.

I have symptoms — can I just wait for my screening invitation?

No. Visible blood, a bowel habit that has changed for weeks, unexplained weight loss or anaemia need a doctor now. Screening is designed for people without symptoms and a negative test does not cancel out a symptom.

Breast cancer screening (mammography)

An X-ray of each breast, read by two radiologists, repeated every two years across a band of ages that starts at 40 in Sweden and 50 almost everywhere else. What it finds, what it misses and what a call-back means.

Cervical screening (HPV test)

Almost every country in the region has switched from looking for abnormal cells to looking for the virus that causes them. The test is better, so the gap between tests is longer — and in several countries you can now take the sample yourself.

Prostate screening (PSA)

Latvia and Lithuania fund PSA testing for men in their fifties and sixties. Sweden assessed the same evidence and recommends against an organised programme. Here is what the trials actually found.

Lung cancer screening (low-dose CT)

The trials are positive and the European recommendation asks countries to build programmes stepwise. In this region it is still pilots rather than invitations — but the eligibility rule is worth knowing.

Abdominal aortic aneurysm scan

The only non-cancer screening programme in the region. Sweden offers men a single abdominal ultrasound in the year they turn 65, looking for a silent widening of the main artery.

Ką patikra gali ir ko negali

Perdiagnozavimas, klaidingai pozityvūs rezultatai, intervaliniai atvejai ir ankstesnio nustatymo laiko iškreipimas – keturios idėjos, esančios už kiekvieno šalių nesutarimo.
